I have never heard of this error on the ACP happening on a localhost, what happens if you shut down the server and then restarted, i have had problems with login in to phpmyadmin before and that is what i have done!

I did all the basic things for solving this problem until backuping(exporting) the data base from phpmyadmin any destroying hole database then creating a new database with same name and importing the exported database.but the result was same. :(

Go to ACP/Styles then under your style in the mainbody, go to details. Make sure all The theme imageset and templates are all correct.

When you "copy" a style, but you don't rename the general information in the style/stylename/style.cfg, then phpbb will still cache the original and not the copy. At least I have had run into this issue in the past.
